People often put the POLST on their refrigerator where medics can see it. It is in four parts:<\/p><p><span class=\"tooltip\"><b>AUTHORITY OF MY AGENT. <\/b><span class=\"tooltiptext\"> Technically this is a durable power of attorney for health care.<\/span> <\/span> This meets the legal requirement for your agent to speak for you when you can&#8217;t speak for yourself. The authority is very broad.<\/p><p><b>INSTRUCTIONS TO MY AGENT.<\/b> It says you\u2019ve discussed your wishes with your Agent and ideally have translated these discussions into a Letter to your Agent. It makes clear that the Agent is to do what you would want, not what the Agent would want for herself or himself. The paragraph gives you an option; you may authorize your Agent to override your wishes as stated in the directive. This issue is really how much do you trust your Agent? And, don\u2019t forget, while you are still able to think for yourself, you can always seek advice from your Agent or from anyone you trust. Indeed, this is often a good idea.<\/p><p><b>INSTRUCTIONS TO MY AGENT AND TO MY PERSONAL AND ATTENDING PHYSICIANS.<br \/><\/b>Because you have lost your mental capacity to make decisions yourself, here is where you direct both your agent and your physicians as to what you expect of them. The first paragraph makes it clear that physicians&#8217; obligation to save life must be subordinate to your desires, which may be to shorten your life.<\/p><p><strong>MY DIRECTIVES.\u00a0<\/strong> There are five choices you may select by initialing. The choices are based on the <b>quality of life<\/b> you wish and not on a particular medical procedure. They are built around the concept that, for the days or months you have left, the quality of your life is more important than is living longer or for as long possible. Choices 3 through 5d are progressive and so there should be no gap in these selections as far as you go<\/p><ol><li style=\"list-style-type: none;\"><ol><li>TIME-LIMITED TRIALS. You may desire time-limited trials to see if such treatments return you to the quality of life acceptable to you, as you have explained to your agent. If you wish to set a time limit for such trails, you may do so on page 3 or on an attached page.<\/li><li>VEGETATIVE STATE. The second choice is to avoid living in a vegetative or near-vegetative condition from which you are unlikely to recover. A vegetative state is a state or condition in which a person is unable to communicate or respond to stimuli despite at times giving the appearance of wakefulness.<\/li><li>DISCONTINUE MEDICAL INTERVENTIONS. <span style=\"color: red;\"><b>The entire form assumes that you have discussed with your agent at some length what the term &#8220;quality of life&#8221; means to you.<\/b><\/span> Moreover, your view of what is an acceptable quality of life may change, broaden or narrow, as your condition progresses. Hopefully you will have been able from time to time to discuss the <b>quality of<\/b> <b>life<\/b> you desire or want to avoid with your agent, before you become incapacitated. As to the length of a time trial, your agent needs to decide that in consultation with your doctors.<\/li><li>ASSISTED FEEDING. This choice means all caregivers must not urge you to eat or drink. In other words, let your body take its course. Your agent must see to this.<\/li><li>STOPPING EATING AND DRINKING (SED). This <span class=\"tooltip\">choice<span class=\"tooltiptext\">Really four choices<\/span><br \/><\/span> is designed around the problem of dementia. However it is applicable to other circumstances as well.<\/li><\/ol><\/li><\/ol><p style=\"margin: 12px 0 12px 0;\">The four choices here could move your agent and other caregivers into uncharted waters. Can your agent and family initiate SED? For a discussion of this issue, see Deep Dive. For this reason, the introductory sentence of choice 5, <b>if checked<\/b>, says choices a through d don&#8217;t apply if the agent and caregivers are at criminal risk. If the sentence is <b>not checked<\/b> and any of a, b, c or d are checked, then your agent and caregivers <b>are<\/b> at risk in most states, except Nevada.<\/p><p>The 5a &#8211; d choices call for SED whenever<\/p><ol style=\"list-style-type: lower-alpha;\"><li>I show no desire to eat and drink,<\/li><li>even if I show a desire to eat or drink,<\/li><li>even if I say, utter, or otherwise indicate that I wish to eat or drink, and<\/li><li>even if I say, utter or otherwise indicate that I wish to live.<\/li><\/ol><p style=\"margin: 12px 0 0 0;\">For page 3 instructions of the form, press &#8220;page 3&#8221; tab, above.MY<\/p><\/div>\n\t\t\t\t\t\t\t\t\t<div class=\"elementor-tab-title elementor-tab-mobile-title\" aria-selected=\"false\" data-tab=\"3\" role=\"tab\" tabindex=\"-1\" aria-controls=\"elementor-tab-content-2603\" aria-expanded=\"false\">Page 3<\/div>\n\t\t\t\t\t<div id=\"elementor-tab-content-2603\" class=\"elementor-tab-content elementor-clearfix\" data-tab=\"3\" role=\"tabpanel\" aria-labelledby=\"elementor-tab-title-2603\" tabindex=\"0\" hidden=\"hidden\"><p>The MISCELLANEOUS PROVISIONS, i.e., Living Arrangements, and Conservatonrship\/Guradianship and After-Death Wishes, are self-explanatory. And they are unrelated to each other.<\/p>\n<p>In the <b>SIGNATURE<\/b> section, you date and sign.  But, do <b>NOT<\/b> do so until you are before those who are observing your signature, typically a notary or two witnesses.
