End of Life Choices California\u00a0<\/strong><\/a>\u2013 End of Life Choices California (EOLCCA) is a nonprofit organization whose trained volunteers provide Californians with information and support as they explore and navigate their legal end-of-life options, including California\u2019s End of Life Option Act.<\/p>

EOLCCA volunteers can provide information about advance care planning<\/strong>, serve as one of the required witnesses to a client\u2019s written request for Medical Aid in Dying (MAiD), and provide trained support and presence on a client\u2019s planned day of ingestion. When requested, a trained volunteer may prepare the MAiD medications and hand them to the client, who must self-administer the medications.<\/p>

All EOLCCA services are provided at no charge<\/strong>. To speak with a trained volunteer, call 760-636-8009<\/strong>.<\/p>

Compassion & Choices<\/strong>\u00a0<\/a>\u2013 A leading nonprofit committed to helping people have the best death possible.\u00a0\u00a0Free counseling, planning, referrals, guidance. State specific forms, and is active in many states lobbying for better end-of-life laws.<\/p>

American Bar Association\/Commission on Law and Aging<\/strong>.<\/a>The\u00a0American Bar Association\u2019s Commission on Law and Aging has an excellent advance directive form that is usable in most states. Remember, when executing (making legal) an advance directive your state\u2019s rules must be followed. See\u00a0Finalization<\/a>\u00a0for more information.<\/p>

Coalition for Compassionate Care of California<\/a>\u00a0<\/strong>\u2013 \u00a0Good information, a workbook, and several advance directive forms, including a simple one.<\/p>

Center for Practical Bioethics<\/strong>\u00a0<\/a>\u2013 \u00a0A very good workbook with a particularly enlightening page on Eating and Drinking including alerts about artificial feeding and IV (intravenous) fluids.\u00a0 Advance health care directive form included.<\/p>

Academy of Aid in Dying Medicine<\/strong>.<\/a> Helps individuals locate physicians who participate in California\u2019s End of Life Option Act (EOLOA). \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 (End of Life Choices California (EOLCCA)<\/strong> also provides information about participating private physicians.)<\/p>

National Hospice and Palliative Care Organization\u00a0<\/strong><\/a>\u2013 \u00a0Its focus is hospice care \u2013 improving care at the end of life.\u00a0 It\u2019s\u00a0Caring Connections<\/strong><\/a>\u00a0website contains advance directives for each state.<\/p>

Final Exit Network<\/strong><\/a> \u2013 (FEN) Core mission is to provide information, education, and emotional support and comfort to competent adults in their own self deliverance from intolerable suffering.\u00a0\u00a0 It does not provide means to end one\u2019s life, nor actively assist in a person\u2019s death. Their attorney is willing to bring legal actions against those who will not honor their Supplemental Advance Directive for Dementia Care.<\/p>

Exit International\u00a0<\/strong><\/a>is unique in the right to die movement in that the organization sees a \u2018good death\u2019 as a fundamental human right, rather than a medical privilege reserved only for the terminally ill.\u00a0\u00a0Subscribers to the \u201cPeaceful Pill<\/i> Handbook<\/i>\u201d can access unique online conversations on end of life (EOL) issues. Exit International gives information on inert gases and particularly on acquiring, assembling and using equipment for nitrogen inhalation.<\/p>
